Medical Home Initiative
Main Street Pediatrics, in affiliation with Children’s Hospital, has developed a Medical Home Model as a way to deliver quality care to our patients. A patient- centered Medical Home calls for care that is accessible, patient and family-centered, continuous, comprehensive, coordinated and compassionate. It is designed to restructure primary care to “comprehensive care under one roof”. This is evidence based care following the AAP guidelines and best practices.
Children with special health care needs are those from birth to 21 years of age who have chronic physical, developmental, behavioral or emotional conditions expected to last 12 months or more and need assistance with coordination of services.
Chronic medical conditions can include:
- Diabetes
- Asthma
- Heart conditions
- Cancer
- Physical disabilities such as MD, CP and spina bifida
- Developmental disabilities such as autism spectrum disorders and mental retardation as well as emotional/behavioral health needs and ADHD.
As a result of your child’s medical history you, may need assistance in coordinating care amongst many providers. My name is Paula Vanesian RN, BSN-I and I am the Medical Home Care Coordinator.
